Application Form
Completely filled out visa application form. If a question does not apply, please write "n/a". Incomplete forms will be returned to applicants unprocessed.
The visa application form should be filled out in legible handwriting or typing.
The purpose of visit must be described in detail.
Photograph
One recent passport-sized (45mm x 35 mm) colour photograph of the applicant, which should be glued or stapled to the marked space at the upper right-hand corner of the application form. The photograph must not be stapled over the facial area. Photographs with staples that overlap the holder's facial features will not be accepted.
Passport
Need to be valid for more than 3 months after the intended departure from Belarus with 1 blank page to affix the visa sticker. British Residence Permit must also exceed by more than 3 months the validity of the requested visa.
Tourist Voucher
An original tourist voucher from a registered Belarusian travel agency. A hotel voucher does not substitute a tourist voucher. Below is the list of registered travel agencies:

Medical Insurance
Medical insurance is required for all foreign citizens traveling to the Republic of Belarus (except British citizens). The insurance certificate can be bought at the Belarusian border. Applicants should not submit their insurance certificate to the Embassy. It should be presented to the immigration officer when entering Belarus.
Letter of Invitation
Only required if visiting friends or family. An original letter of invitation (not a copy) made up by the applicant's friends or relatives residing in Belarus at the local passport and visa office of the Ministry of Internal Affairs.
An invitation may also be submitted in person by a Belarusian citizen on a temporary visit to the United Kingdom. The signature on this invitation must be notarized at the Consular and Visa Section of the Embassy of Belarus (the consular fee is £20).

If an application is made by post then a prepaid and self-addressed return envelope should be enclosed.

General Visa Information:
Within 3 business days after arriving in Belarus visitors should report the information about the address and duration of stay to the local police station so that a registration stamp is put in their passport. If staying in a hotel, this will be arranged by the hotel services.
