|
Return ticket or the applicant's driving license plus technical license along with a confirmation of insurance (green card) or Original voucher issued by travel agency verifying payment of services in the Slovak Republic (length of stay, hotel accommodation, board and lodging, trips, etc.)

Financial Means
|
Bank card with statement confirming recent balance on the card, recent bank statement confirming the applicant's income of last 6 or 12 month including month when the application is submitted, traveller's cheques or cash. The minimum sum required by Slovak authorities for is GBP 41, or equivalent in local currency, per person per day to be spent in Slovakia or the Schengen area.

Status Proof
|
A recent original letter from the employer, solicitor, Company House or for students a recent original letter from their school, college or university.

Travel Insurance
|
Letter from the insurance company stating that the applicant will be covered for any medical expenses, hospitalization and repatriation for at least the equivalent to EUR 30,000 during their stay in Slovakia. Name, validity of insurance and amount which covers medical expenses must also be stated in the letter.

Letter of Invitation
|
Required only if the applicant has been invited by a private person or visiting friends and family. An original letter of invitation signed by the host in Slovakia, the signature must be verified by a Slovak notary or by the Embassy of the Slovak Republic.

Self addressed mail
|
If the applicant desires the visa to be returned by mail then a stamped, self-addressed envelope with postage stamps for "signature required mail or special delivery envelope is to be provided.
