Application Form
|
Completed and signed application form with 1 recent passport size photo.

Passport
|
Need to be valid for more than 3 months beyond the validity of the requested visa, with a blank visa page to affix the visa sticker. Also a photocopy of your passport (only the data page with photograph, signature, passport validity and UK Residence Permit).

Accommodation
|
Confirmed hotel reservation for the entire stay or confirmation of tour.
|
Transportation
|
Confirmation of airline, rail or bus ticket. If driving then ferry confirmation, registration document, proof of legal ownership of vehicle and insurance papers are required.

Financial Means
|
Latest and most up to date bank statement showing a positive balance or traveller cheques worth £30 per person per day to be spent in the Schengen area. Cash and Credit card statements are not accepted.

Status Proof
|
A recent original letter from the applicants employer, solicitor, Company House or Chamber of Commerce or for students a recent original letter from the applicants school, college or university. Pay slips, work contracts and Student ID cards are not accepted.

Travel Insurance
|
Minimum of EUR 30,000 medical coverage including comprehensive accident, emergency hospital treatment and repatriation valid for all Schengen countries. Its validity must exceed validity of the visa. The insurance document should mention name of the person, period of cover and also include the terms and conditions of the travel insurance.

Letter of Invitation
|
Required only if the applicant has been invited by a private person or visiting friends and family. The letter from the inviter should state who they are, whom they are inviting, what the link between them and the visa applicant is, why they are inviting the individual in question, for how long, where the applicant guest will stay (in their home) and so on. The letter must provide the embassy or consulate with full details of the planned visit.

Visa Application Procedure:

All applicants need to apply in person at the Czech Embassy in London between 9-12hrs Monday to Friday.
Copies of all supporting documents are to be provided.
Family members of EU Citizens can apply either in person (without prior appointment) at the Czech Embassy in London from 11:00 am to 12:00 pm or by post.

Visa Collection Procedure:
|
Passports can be collected between 13:15-15:00hrs Monday to Friday.
Passport returned by mail if the applicant includes a pre-paid self-addressed Special Delivery Envelope during the submission of the application.

General Visa Information:
|
Foreign nationals whose stay in the Czech Republic will exceed 30 days are obliged to register with the Alien and Border Police within 30 days on their arrival in the Czech Republic.
If staying in a hotel or similar institution the provider of the accommodation should arrange this registration.
